You don’t need to be a professional to make a referral to Outcomes Lab. This form can be completed by the person seeking support or by someone they trust, like a family member, friend or support coordinator. We offer both private and NDIS-funded services, with a focus on respectful, inclusive and person-centred care.
Outcomes Lab acknowledges the Traditional Owners of the land on which we work, Bunurong/Boon Wurrung and Wurundjeri Woi Wurrung peoples of the Eastern Kulin Nation.
